After creating a parent and clicking save TWICE i get this error

Thank you for reporting the error.
Are you able to reproduce it on the demo? (You may have to create User Fields first, just like on your instance).

Yes Same Error when I tried to save the demo Parent and Teacher, on investigation I noticed that the Staff Table is missing the field custom_200000003 I could alter the table staff to add this column but I dont know the format for the field.

Thank you for your answer.
Let's proceed this way:
Could you please try to delete the User Field with ID 200000003 ?
You can get the field ID this way:
Go to menu Users > User Fields, then pick "General Info". Then click on the fields in the list:

As you can see in the URL bar of your browser, the URL ends with the field ID, on the screenshot it is id=200000013.
When you have spotted the field with ID 200000003, click "Delete".
